Multiple myeloma

Multiple myeloma (MM) is a challenging blood cancer, particularly in relapsed/refractory (R/R) cases. While therapies like proteasome inhibitors, immunomodulators, CD38 monoclonal antibodies, and stem cell transplants have improved outcomes for newly diagnosed patients, treatment-resistant forms of MM remain a serious concern. A new therapeutic approach combining two targets, BCMA and GPRC5D, may provide a breakthrough solution.

BCMA has long been a key target for treating R/R MM due to its expression in malignant plasma cells. However, BCMA-targeted therapies face limitations, including tumor cells losing or downregulating BCMA, leading to disease recurrence. This is where GPRC5D, a protein highly expressed in MM cells and linked to poor prognosis, comes in. By targeting both BCMA and GPRC5D, researchers aim to overcome the limitations of single-target therapies.

A collaboration between a leading Chinese hospital and research institutions has now delivered promising results. A Phase I clinical trial involving 21 patients with R/R MM using the dual-target BCMA/GPRC5D CAR-T cell therapy reported an overall response rate (ORR) of 86%, with 75% of patients achieving a complete response (CR). Notably, even patients whose cancer cells lacked BCMA or GPRC5D expression showed significant improvement, underscoring the versatility of this treatment.

**Key Results from the Study:**

– 21 patients with advanced, heavily pre-treated R/R MM participated.

– Of the 12 patients who received the optimal dosage of 2.0×10⁶ CAR-T cells/kg, 86% showed clinical response, with 75% achieving complete remission.

– Importantly, patients with BCMA or GPRC5D-negative cancer cells also responded well to treatment.

– The treatment was well-tolerated, with manageable side effects, including 71% of patients experiencing mild to moderate cytokine release syndrome (CRS).

This trial, published in *The Lancet Haematology*, represents a major step forward in MM treatment. The dual-target approach addresses the limitations of BCMA-targeted therapies, offering new hope to patients who have exhausted conventional options.

💪Meet John, a resilient man from New Zealand who has been battling multiple myeloma, a relentless form of cancer. Severe bone pain has confined him to bed for an extended period, disrupting his daily life. John’s journey began eleven years ago when he was diagnosed with multiple myeloma. According to the American clinical trial database Clinicaltrials, as of November 10, 2023, China has taken the lead in CAR-T cell therapy clinical research with a total of 655 studies, marking the country’s first foray into the international forefront of a new drug development field. Among them, Legend Biotech’s SicajioLunsei and Koji Pharmaceutical’s Claudin18.2 CAR-T have both achieved global leadership. Additionally, several companies are also advancing next-generation CAR-T and allogeneic CAR-T technologies, showing promising initial data and potential for global market success.

In terms of pricing, all three Chinese CAR-T therapies cost less than 1.3 million RMB (approximately 180,000 USD), only one-third of Novartis’s Kymriah. Specifically, JW Therapeutics’ Beinoda (Relmacabtagene autoleucel) is priced at 1.29 million RMB, FOSUNKITE’s Yikaida (Yescarta) is priced at 1.2 million RMB, and IASO’s Fukesu (Equecabtagene ciloleucel) is priced at 1.166 million RMB. Furthermore, Juventas, a domestic Chinese CAR-T product developed by Hegen Biotech, has set a new milestone in the pricing of Chinese CAR-T therapy, priced at 999,000 RMB per dose (less than 140,000 USD).
The year 2021 marked the beginning of cellular immunotherapy in China, with CAR-T therapy leading the way as one of the future development directions. CAR-T therapy primarily involves modifying T cells to recognize specific targets on tumor cells. Theoretically, there could be countless CAR-T therapies targeting different targets, which means endless possibilities.
We hope that in the near future, with the efforts of medical researchers both domestically and internationally, we can reduce the side effects of cell therapy, lower prices, and break through the bottleneck of solid tumors, benefiting more and more late-stage cancer patients!

China’s Chimeric Antigen Receptor (CAR) immunotherapy is making significant strides, spearheading innovation in breast cancer treatment. 🇨🇳💪
 
Recent studies indicate substantial breakthroughs in CAR-T therapy, as well as CAR-NK cells and CAR macrophages for breast cancer treatment. China is at the forefront of global clinical research on CAR, with 16 clinical trials focused on CAR-T or CAR-NK cell therapies currently underway. ⚡️⚡️Lyn, a mother of three from Singapore, faced unexpected health challenges in 2022. She experienced persistent pain in her shoulders, neck, and upper limbs, disrupting her sleep and even affecting her ability to breathe. A visit to the emergency department at Shanghai Jiahui International Hospital revealed a shocking discovery – a massive tumor in her mediastinum, measuring 15 cm in diameter! The compression from this large mass not only caused excruciating pain but also impacted nearby organs such as the heart, major blood vessels, and lungs, leading to Superior Vena Cava Syndrome (SVCS). Lyn struggled with breathing difficulties, posing an imminent threat to her life.

🌸🌸The diagnosis pointed to “Primary Mediastinal Large B-cell Lymphoma,” a relatively uncommon subtype of lymphoma, prevalent among young individuals, particularly females. Despite undergoing various comprehensive treatments, including chemotherapy combined with targeted therapy and consolidative radiotherapy, Lyn unfortunately showed no response to these treatments. Eventually, the decision was made to pursue CAR-T cell therapy.

🧶🧶Lyn’s husband emphasized, “The cost of CAR-T treatment abroad is significantly higher than in China, and the waiting period is at least 3-6 months. We trust the medical expertise of China and Shanghai Jiahui. Their professionalism and meticulous care left a deep impression on us. Our decision was absolutely correct.”

💥💥 The treatment team at Shanghai Jiahui International Hospital, led by Director Vicky H. Lee and Dr. Lili Zhou, the head of the Hematology and Oncology Department, developed a meticulous treatment plan to navigate Lyn through the entire CAR-T therapy. They highlighted the critical importance of two stages: the period from cell collection to pre-infusion and the phase post-infusion, which poses a risk of Cytokine Release Syndrome (CRS) and Immune Effector Cell-Associated Neurotoxicity Syndrome (ICANS).

🍁🍁 After careful preliminary treatments, infection screening, and confirmation through cell collection, Lyn received the CAR-T cell infusion. Following infusion, the “T-cell soldiers” proliferated successfully in her body, providing relief from tumor-related pain. Upon clinical assessment confirming stable conditions, Lyn smoothly transitioned to being discharged.

⛵️⛵️ It’s worth noting that CAR-T therapy costs comprise drug and non-drug expenses, with the overall treatment expenses significantly lower in China compared to Western countries. Shanghai Jiahui International Hospital collaborates with local high-quality medical resources such as Ruijin Hospital and Tongji Hospital, offering patients a comprehensive one-stop CAR-T diagnostic and therapeutic service.
